You tell us what’s happening and what you need. We put together a plan built for your situation. Then we stay in touch while you work it — paperwork, providers, the move itself, the month after.
A short form, then a 30–45 minute call. Your state, your care, your legal status, what you have to work with, what’s in the way. No pressure on how much to share.
A personalized brief: destinations worth considering, care providers, Medicaid and SSDI transfer steps, name and gender-marker process in the destination, housing and mutual-aid leads, a checklist for the next thirty days.
Weekly check-ins while you work the plan. Provider referrals, court filings, coverage appeals, housing leads. Follow-ups at thirty, sixty, and ninety days after you arrive.
